Salary: Grade H1 or H2 SCP 27 - 35 £37,035 - £44,711 pro rata per annum

We have an exciting opportunity for a 12 month temporary maternity leave cover Social Worker post for 22.2hrs ( 3 days per week) to become a part of our Discharge to Assess Social Care Team. The successful candidate will join a well-established and supportive team of Social Workers, Case Assessors and Support Workers; who work together with multi- disciplinary colleagues to ensure effective health, care and support assessments and services are implemented for people who have recently been discharged from hospital.
You will need to be passionate about supporting our community in Blackpool, be committed to your role as a Social Worker and keen to grow and develop in line with Social Work England protocols.
