We have an interesting opportunity for an enthusiastic and organised individual to work as a temporary Case Assessor for up to 12 months in a local community setting within Adult Social Care. The role involves meeting individuals who require support and advice about services available to help people to maximise their independence by the use of statutory provision (Care Act 2014) and also the voluntary sector. It also involves working with other professionals (Health and Social Care) to help people remain in their own homes for as long as possible.
