Blackpool's Adult Services has well established joint working partnerships across multi-disciplinary settings supporting the health and social care field.
Our team is made up of a variety of dedicated staff, including managers at all levels, social workers, case assessors and support workers.
Team members work alongside a wide range of health colleagues in teams including wellbeing workers, nurses, therapists, community matrons, specialist nurses, dieticians, mental health professionals and GP's. We work really hard to support the health and wellbeing of the residents of Blackpool by focusing on hospital discharge, preventing readmission into hospital, offering rehabilitation, intermediate care, crisis support and reablement.

Working in adult social care can be a varied role, with a responsibility that can really make a difference to our local people. This area of social care sits at the heart of the local Integration Agenda. The work is often fast paced and pressured. Within our staff group we support ASYE Social Workers, providing support to students via Practice Educators across all settings. We pride ourselves on offer ongoing support from a managerial perspective and also from experienced staff who have a good knowledge of the sector.
When you join our team, we can offer a wide range of experiences across the field. This can include shadowing other experienced Social Workers within our team who are based in local hospitals, Accident and Emergency, the local Hospice, Rapid Response Services and Discharge to Assess Teams to give a holistic overview of health and social care partnership.
