The Community Home Care and Reablement Service is looking to recruit a highly motivated individual to undertake a Team Leader position. Working 37 hours per week, between the hours of 7am and 10.15pm including weekends on a rota basis. The role includes supervising and managing a community based team of staff who provide a range of urgent care, Rapid Response, hospital discharges, and reablement provision for individuals needing care and support within their own homes.
We are looking for an extremely motivated, dynamic individual with a wide range of skills and experience who can work as part of a team and on their own initiative with the ability to adapt to change in a fast moving, innovative and customer focused service. The ability to build and maintain excellent relationships with all stakeholders of the service is also critical.
The ideal candidate will be exceptional at providing the leadership needed to develop and deliver the future vision of the service whilst acting as an appropriate role model. In addition the successful candidates need to be able to work under pressure and ensure that records are accurate and kept up to date.
