We are looking for highly motivated, dynamic people with a wide range of skills who are reliable and hardworking and can work as part of a team and are able to adapt to change in a fast moving, innovative and customer focused service.
Your role will be to help vulnerable people maintain and regain their independence and remain in their own home with an improved quality of life. The role also involves supporting individuals upon discharge from hospital as part of the Discharge to Assess pathways and Complex and Palliative care.
The successful candidates will be working on a rota, including weekends, bank holidays and shift working. This is an excellent opportunity for the right people to work in a modern, caring, fulfilling service, in return we offer excellent staff support systems, training and development. Although previous experience could be an advantage, it is not essential. We are able to be flexible in the contracted hours offered and preferences can be discussed at interview.

A full driving license and use of a car is preferable.
